Liability Release Form - Pickleball Clinic Liability Release Form - Pickleball Clinic Anyone who wishes to participate in pickleball at HOLY FAMILY CATHOLIC CHURCH, must sign this release form Diocese of Nashville Hold Harmless/Indemnity Agreement ACTIVITY PARTICIPANT First Last Last Email The above named ACTIVITY PARTICIPANT agrees to defend, protect, indemnify and hold harmless, the HOLY FAMILY CATHOLIC CHURCH and THE DIOCESE OF NASHVILLE against and from all claims arising from the negligence and or fault of the above named ACTIVITY PARTICIPANT or any of their agents, family members, officers, volunteers, helpers partners, organizational members or associates which arise out of the activity of PICKLEBALL associated with HOLY FAMILY CATHOLIC CHURCH. Additionally, the above named ACTIVITY PARTICIPANT agrees to protect, defend, hold harmless and fully indemnify the HOLY FAMILY CATHOLIC CHURCH for any claim or cause of action whatsoever arising out of the activity of PICKLEBALL which takes place during the stated dates and times of PICKLEBALL that is brought against HOLY FAMILY CATHOLIC CHURCH by the above named ACTIVITY PARTICIPANT or their family members whether such claim arises from the alleged negligence of HOLY FAMILY CATHOLIC CHURCH, its employees or agents or PICKLEBALL LEAGUE negligence.
